임플란트와 mini-screw를 이용한 구치부의 수복

Abstract
하악 구치부의 상실은 임상에서 흔하게 볼 수 있으며, 상실 후 적절한 수복치료가 이루어지지 않으면 상악 대구치의 정출을 동반하게 된다.
구치부 수복을 위해 인접 지대치의 삭제를 통한 고정성 보철물을 시행할 수도 있으나, 치아의 보존적, 기능적, 치주적 측면에서 단일 치아의 수복을 위해서는 임플란트 사용이 권장되어 일반화되고 있으며, 그 성공률 또한 매우 높은 것으로 보고되고 있다.1,2)
또한 이러한 이유로 임플란트를 원하는 환자의 경우, 정출된 상악 구치를 치관의 삭제나 근관치료, 보철 치료를 통해 하악 보철물의 수직공간을 확보하는 것은 환자나 의사 모두에게 납득하기 힘든 술식이 될 것이다. 과거 구치부의 압하는 매우 어렵다고 판단되어, 복잡한 장치나,3,4) 수술을 동반하여5,6) 일부 시행하였으나 근래 mini-screw의 도입7,8)으로 비교적 쉽게 시행할 수 있게 되었으며, 그 성공률과 안정성이 매우 높은 것으로 보고되고 있다.9,10,11)
임플란트 매식 후 3-4개월, 또는 5-6개월의 골 유착 유도 기간12)을 이용하여 정출된 대합치의 압하를 시행하게 되면 전체적인 치료기간의 증가없이 보철 치료가 가능하다. 즉, 임플란트와 mini-screw의 이용은 보철 수복에 있어 기능, 심미, 보존, 치주적 측면 뿐 아니라 환자 및 의사의 편의와 만족을 극대화시킬 수 있다.
The loss of mandibular molars with extruded antagonists is a common problem in prosthodontic patients. In most cases, orthodontic and prosthodontic treatment is necessary for restoration of missing molars and surgical or endodontic approaches are needed as well.
Osseointegrated implants and non-osseointegrated implants (orthodontic mini-screws) can simplify the treatment protocol for these complicated problems. In addition these implants can also maximize efficiency in the operation, patient comfort and result satisfaction without additional periods of treatment or damages to adjacent or opposite teeth for prosthodontic restoration.
